To begin, locate the Evaluation Form by clicking the “Get Started” button on our website’s home page. Alternatively, you can find the form in the drop-down menu, accessible by clicking the Menu button in the top right corner of the current page.
On the Evaluation Form page, please enter your first and last name separately, along with your preferred email address.
After submitting your name and email, check your inbox for a confirmation email. Click on the link provided in the email to confirm your email address.
Once your email is confirmed, return to the Evaluation Form page. Complete the form, upload any required pictures, and submit your evaluation.
